Housing Request Form
This form is to be used to either:
- Request on campus housing, or
- Request a change of housing assignment
- You will automatically receive a copy of the information that is being submitted on this form.
- Students living off-campus and assigned on-campus housing will be assigned a mandatory meal plan depending on the building assigned.
- Any questions on billing can be directed to the housing office by emailing firstname.lastname@example.org.
- Students cannot be guaranteed a specific assignment or roommate
- If granted, students will be required to be financially clear for the space they are requesting.
- Students will not be permitted to move their items into their new room until they are financially clear. Instructions for this will be sent to student.
- Students must email their acceptance of their room change within 24 hours of being offered this change and understand that failure to do so will result in cancellation of their change.
