All incoming first-year and transfer students should have received an Entrance Health Certificate/Immunization Form in their Wentworth acceptance packet. If not, please click here to download a copy.
The student and his/her health care provider must complete the form, including information about immunizations as required by State law. By August 1 please mail the completed form to:

Students whose immunizations records are incomplete may be prevented from registering for classes.
